Privacy Policy
Effective August 7, 2026. This policy explains how TrueNorth Omni Care handles your information on truenorthoc.com and in the TrueNorth mobile app.
TrueNorth Omni Care ("TrueNorth", "we", "us") respects your privacy. This Privacy Policy describes the personal information we collect, why we collect it, how we use and share it, and the choices and rights you have. It applies to our website, our patient portal, and the TrueNorth mobile application distributed through the Apple App Store and Google Play (together, the "Services").
1. Information we collect
Information you provide. Name, date of birth, email address, phone number, mailing address, insurance or Medicare details, emergency contact, and any message or form content you submit (for example, a membership application or a callback request).
Health information. If you become a patient or member, we may collect medical history, medications, allergies, visit notes, lab results, and care-plan information. This information is Protected Health Information ("PHI") and is handled under HIPAA and applicable state law.
Account and device information. Login credentials, app version, device model, operating system, language, crash diagnostics, and a device identifier used to deliver push notifications.
Usage information. Pages and screens viewed, features used, referring URL, approximate location derived from IP address, and timestamps.
We do not knowingly collect information from children under 13, and the Services are not directed to children.
2. How we use your information
To provide and coordinate your care; to schedule, confirm, and follow up on appointments; to verify eligibility and process membership; to operate, secure, and improve the Services; to respond to your questions; to send service, appointment, and care-related messages; and to comply with legal, accreditation, and billing requirements.
We do not sell your personal information, and we do not use PHI for advertising or third-party marketing.
3. Legal bases and consent
We process information to perform our agreement with you, to comply with legal obligations, for our legitimate interest in operating a safe and reliable service, and — where required — with your consent. Where consent is the basis, you may withdraw it at any time without affecting prior processing.
4. How we share information
We share information with clinicians and care partners involved in your treatment; with insurers and payers for coverage and payment; with service providers (hosting, secure messaging, analytics, communications) bound by contract and, where applicable, a Business Associate Agreement; with authorities when required by law or to prevent serious harm; and in connection with a merger or acquisition, subject to this policy.
5. Data retention and security
We retain medical records for the period required by California law and applicable federal rules, and other data only as long as necessary for the purposes described here. We protect information with encryption in transit and at rest, access controls, audit logging, and staff training. No system is perfectly secure, so we cannot guarantee absolute security.
6. Your rights and choices
Depending on where you live, you may request access to, a copy of, correction of, or deletion of your personal information; restrict or object to certain processing; request portability; and appeal a decision. California residents have rights under the CCPA/CPRA and, for medical records, under the CMIA. You may opt out of marketing emails at any time using the unsubscribe link. We will not discriminate against you for exercising your rights.
To exercise a right, email privacy@truenorthoc.com. We may need to verify your identity before responding.
7. Account and data deletion
You can request deletion of your TrueNorth account and associated app data at any time from the app's Settings screen or by emailing privacy@truenorthoc.com. We will delete or de-identify your data within 30 days, except records we must retain by law (such as medical and billing records).
8. Mobile app disclosures
The TrueNorth app requests permissions only for features you use: notifications for appointment reminders, camera and photo access to upload documents, and optional biometric unlock handled entirely on your device. We do not use third-party tracking for advertising and do not link data collected in the app to third-party data for tracking across apps or websites. You may change permissions at any time in your device settings.
9. International transfers
The Services are operated from the United States. If you access them from outside the U.S., your information will be transferred to and processed in the U.S. under appropriate safeguards.
10. Changes to this policy
We will post any updated policy on this page with a new effective date and, for material changes, notify you in the app or by email before the change takes effect.
